APPLICATION FOR A FORM I-20


Thank you for completing this Application for a Form I-20. Please note that the following items must be received before we will issue a Form I-20:

  • Upon the student's acceptance to Academy at the Lakes, you must submit the Online Enrollment Packet & Continuous Enrollment Agreement by the date listed on your child's acceptance letter. Enrollment Instructions are sent via email. Please contact the Admissions Office if you have questions regarding the Online Enrollment Packet & Continuous Enrollment Agreement.
  • Submit this Application for a Form I-20 
  • Submit the Affidavit of Financial Support and provide supporting documents (a link to this form is provided within this application).
  • Submit the Affidavit of Free Room and Board and Proof of Financial Capability, if applicable (a link to this form is provided within this application).
  • Submit the Power of Attorney, if applicable.
  • Provide a copy of your child’s passport
  • Provide Your child's immunization record
  • Provide Proof of health insurance

The I-20 is the first step to obtaining an F-1 Student Visa. Families must make an appointment with their local United States consulate or embassy to obtain an F-1 student visa. International students must have an F-1 student visa before they can begin classes. If you are unsuccessful in obtaining a student visa, please refer to our Visa Denial Refund Policy.

The Department of Homeland Security requires that F-1 students complete a “school transfer” when beginning studies at a new school. Please email admissions@academyatthelakes.org for a copy of the School Transfer Form.

Does your child plan to travel outside the U.S. before school starts?*

If your child is currently on an F-1 visa, they will receive a school transfer form or other instructions. If your child holds another status, we would like to speak with you before issuing your child’s Form I-20.

If your child attended a U.S. school last term and is home on holiday between academic terms, your child is considered a “School Transfer” and needs to have their SEVIS record transferred to us before a Form I-20 may be issued. Please contact your child’s international student adviser at their previous school to authorize the release of your child’s SEVIS record to Academy at the Lakes.

PART II: Receiving the Form I-20

Part III: Annual Financial Requirements for International Students for the 2022-23 School Year (Estimated)

Average living costs for a 10-month academic year are estimated below. This is a modest, “no extras” budget. It is highly recommended that your child bring at least 10% more.

*This includes local transportation only. Travel to and from your child’s country or Morning Transportation service offered by Academy at the Lakes are additional expenses.

While we attempt to make the tuition at Academy at the Lakes all inclusive, there are some additional charges that may apply to your student(s). In an effort to help you plan for the  year, the following is a list of additional expenses that are not represented in the chart above:

  • Required International Student Fee: $2,500 per student
  • Required $600 Capital Assessment Fee per student is billed, annually.
  • A one-time matriculation fee of $500  is billed separately for new students, only.
  • Extended Care (K – 8th)
  • Extended Trips (5th – 12th)
  • Morning Transportation (Optional)
  • Lunch Program (Optional)
  • Uniforms (K – 8th)

Additional expenses outside of tuition (for example extended trips, AP exams, extended care, after-school activities, student club dues, SAT prep) will be billed separately.

If the student will live with someone other than a parent while enrolled at Academy at the Lakes, please provide the following documents:

  1. Power of Attorney - This is a legal document to be completed by the student's parent(s). It must be notarized.
    • If translations are required for a proper legal signature, you are required to get this document translated and signed.
